Schedule An Appointment

 

 

You may schedule an appointment by calling our office at (262) 782-5141.  When you make an appointment with us, we are reserving a specific time for you.  In order to remain on time for all of our scheduled patients, we kindly ask that you arrive on time for your scheduled appointment.

 

If this is your first visit to our office,  please arrive 15 minutes prior to your scheduled appointment to complete new patient paperwork.  

You may also download the forms from the website and bring the completed forms with you to your first visit.

 

If you need to reschedule or cancel your appointment, we require a 48 hours notice to avoid a cancelation fee*.  If calling after business hours, please leave a detailed message including your name, phone number and the date and time of your appointment.

 

*Arriving late may result in a rescheduled or a follow-up appointment.
